We want to make it easy to contact us regarding any of your billing questions, issues or cancellations. There are additional forms that can help with your billing needs: Change of Bank Draft Form, Change or update Credit Card Form, and Secure Email Upload where you can send back forms securely to ensure you keep your information private.
UBA Membership and all optional supplemental UBA Gap Membership Products that include group insurance are a monthly subscription. You will continue to be drafted every month until you cancel by either using the cancellation form below, emailing us or calling us. We showcase our name UBA GAP and our number 866-438-4274 on all transactions (shown as UBAGAP8664384274) on your account statement, and it is your responsibility to check the transactions occurring on your account every month and to cancel with us when desired. Every month we pay for the membership services and the insurance premiums for any applicable group insurance programs on your behalf, whether you use the membership services or file a claim with the group insurance programs. Please refer to our Refund Policy for details on refunds. Please note that UBA Billing accepts Bank Draft monthly billing or Credit Card monthly billing (MasterCard, Visa & Amex). We do not do any direct mail billing.
Important: If there is an issue with a draft on either Bank Draft or credit card, you will receive an email requesting you to Contact Us in order to get the payment issue resolved. Please make sure to respond to these emails on a timely basis to avoid any lapse in membership and group insurance coverage.
When you change from credit card to bank draft, or you want to use a different account than listed on your original application, please use this form below in order to submit the request. Once you have the form completed, save it to your computer and then attach it using the Send Forms Back Securely link to send it encrypted and to protect your information. Make sure to get the form turned in before your next bill date to avoid any lapses in your UBA product(s).
When you change from bank draft to credit card, have a new credit card number, new expiration date and CVV code, please use this form below in order to submit the request. Once you have the form completed, save it to your computer and then attach it using the Send Forms Back Securely link to send it encrypted and to protect your information. Create a reminder to update with UBA when your credit card expires to avoid lapses in your UBA product(s).
